Found in 1955 in the so-called "house of Proclus". It was in second use in the domestic shrine embedded into a wall together with the relief ΝΑ 1955 ΝΑΜ 12 and the funerary table (mensa) ΝΑ 1955 ΝΑΜ 90.

The relief is in the form of a temple with antas and roof antefixes. In its interior depicted is a bearded god wearing a himation. He seats on a throne, rests his feet on a footstool and holds a cornucopia and phiale. He is a chthonian deity identified either with Asklepios or Zeus Meilichios or Pancrates.

Towards the god walk three figures, a child that drives a ram to sacrifice, a bearded man and a woman from whom only the head is preserved.
